2. Choose from the following appointment options

→ On-site location (face-to-face)

→ Telephonic

10/1 Acquisition - Onsite Appointment: Crane, Hogan Park and Midland locations ONLY Select the onsite location (Crane, Hogan Park, Midland) from the drop-down menu above if you wish to meet onsite with a benefit counselor for education on benefit options and assistance with completing your 10/1 benefits enrollment.

or select the "10/1 Acquisition - Telephonic Appointment" option if you wish to speak with a benefit counselor over the phone for your benefits appointment.

10/1 Acquisition - Telephonic Appointment (Any 10/1 Acquisition Location): Select this option to schedule an appointment to meet with a benefit counselor telephonically for education on benefit options and assistance with completing your 10/1 benefits enrollment.

New Hires (excludes 10/1 Acquisition employees): ONLY select this option if you are a NON 10/1 Acquisition new hire and electing benefits for the first time. This option is found at the very bottom of this drop-down list.
